[01] Notice packet

A copyright owner or authorized agent should prepare a signed written notice containing:

  1. Identification of the protected work, or a representative list for multiple works.
  2. Identification and a precise URL location for the material claimed to infringe.
  3. The claimant’s name plus reliable postal address, telephone number, and email address.
  4. A good-faith statement that the disputed use is not authorized by the owner, agent, or law.
  5. A statement under penalty of perjury that the notice is accurate and the signer is the owner or authorized agent.
  6. A physical or electronic signature.

An incomplete packet may prevent or delay evaluation. Knowingly making a material misrepresentation can produce legal liability.

[02] Counter-notification packet

A provider who believes material was removed or disabled through mistake or misidentification may prepare a signed counter-notification identifying the material and former location, giving complete contact information, and stating under penalty of perjury a good-faith belief in the mistake. It should include the jurisdiction and service-of-process consents required by applicable law.

Where appropriate, a valid counter-notification may be sent to the original claimant. Restoration may occur after the statutory interval unless the claimant reports an action seeking a court order.

[03] Repeat-infringer rule

In appropriate circumstances, access associated with repeat infringement may be restricted or terminated. Material may also be disabled where a substantiated claim or legal duty supports action.

[04] Routing warning

The Report Content form is a local worksheet and cannot deliver a DMCA notice or preserve a deadline. The Contact Us form is likewise local-only. Aggregate review outcomes are listed in the Transparency Report.
